NEW
PATIENT INFORMATION
We
will mail you a new patient packet. Please complete all the
enclosed paperwork and bring it with you to your first office visit.
Also,
please remember:

Bring ALL Medication bottles with you.
- •
Medical Records: We need your past medical records. If you have
them, please bring with you. If you do not have a copy of your records,
you will need to sign a release in order for us to obtain them for
you.

Bring your Insurance Cards with you, so that we may make a copy
for your chart.
- •
Co-payment: If your insurance requires a co-pay you need to be prepared
to pay that amount when you check out.
- •
Payment
in full for SELF PAY patients unless arrangements are made prior
to your appointment…you may call the billing office for assistance

Prescription
refills
Please
ask for any medication refills during your office visit. If you
have an urgent need for a refill, please call during office hours.
We discourage prescriptions after hours because your chart and
history are not readily available to our physicians.
Calling
our office
We
encourage you to ask questions. If you have a problem with your
medication, a change in or any unusual symptoms, or a problem
for which you cannot wait until a scheduled appointment, please
call our office between 8:15 am and 4:45 pm. One of our staff
members will speak with you or return your call.
After
hours

At
least one of our physicians is on call 24 hours a day. If you
find you must speak with a cardiologist after office hours or
on weekends, please call our office number. Our answering service
will ask for your name and the nature of your problem and will
forward your message to the physician on call.
Emergencies
If
you have a cardiovascular emergency, we recommend that you go
to the nearest hospital emergency department or call 911 for transportation.
The emergency department will contact the physician on call. Conditions
that require emergency evaluation include, but are not limited
to, chest pain, severe shortness of breath, or passing out.
